Did you delete the Continue button on the Score Slide? That is a bad idea: the actions specified in the Quiz Settings, both for Success and for Failure will be executed only when this Continue button is clicked. You have to be very careful with deleting objects on question/score slides that are embedded in the slides because most of them have their functionality and if you delete them you lose it. Can you give more details about your work flow?
